Crisis Case Manager Duties include but are not limited to:

  • Crisis case management, consultation, community liaison work and EHR documentation.
  • Collaborate with the treatment team on treatment planning, planning provision, coordination of services, discharge planning and evaluation of services.
  • Successfully complete all required trainings.
  • Provide support, education and service coordination to significant others on behalf of the children presenting for service.
  • Produce client related service records and maintain accurate and up to date documentation of EHR consistent with agency policies and procedures.
  • Perform other duties as assigned by supervisor which are consistent with the position and in compliance with agency policies and procedures.
  • Receive ongoing supervision from the Director of Family Stabilization and work collaboratively with the Crisis Therapist and Psychiatric staff.
  • Home and community visits, as needed.
  • Maintain licensure according to professional guidelines.
  • Attend agency and team meetings required. Participate in all team planning, case reviews, and program achievement.


Crisis Case Managers Qualifications:

  • Possess a Bachelor’s/Master’s Degree in Human Services or related field.
  • Have prior experience working with children and families providing supportive services, crisis intervention, and family development for at-risk families.
  • Possess experience in utilizing community resources.
  • Ability to effectively communicate with multi disciplines associated with community resources and possess the skills to work as a team member.
  • Ability to reasonably problem-solve complex mental health client problems and needs.
  • Effective time management and interviewing skills.
  • Awareness of the service populations’ cultural and socioeconomic characteristics.
  • Have A valid Ohio’s driver’s license and proof of insurance.
  • Pass a criminal records check.

Outpatient Therapist - Contract

  

Benefits:

  • Create your own case load (minimum 5)
  • $30 per hour for CPST/Consultation services
  • $50 per hour for Counseling sessions


Duties include but are not limited to:

  • Interviews and completes psychosocial intake diagnostic assessments with children and families.
  • Counsels clients after selecting therapeutic approach to use with individuals, families and/or groups with clearly established measurable goals/objectives, frequency, intensity, location and duration of treatment.
  • Provides therapeutic interventions both in and out of client’s home on a planned and/or emergency basis to: effect improved mental health adjustments, aid clients to mobilize inner strengths and environmental/social resources to improve social functioning, and serve hard to reach or resistant clients and families.
  • CPST responsibility as delegated by primary case manager.
  • If qualified, may administer and interpret intelligence, achievement, interest, personality and other psychological test to diagnose disorders and formulate plans of treatment.


Minimum qualifications:

  • Bachelor’s Degree, LSW/LPC (work at our home location is required).
  • Master’s Degree, LISW/LPCC and/or dual licensure preferred (remote or office location work available).
  • Experience in treating children, adolescents and families individually or in a group setting.
  • Knowledge & skills in need and identification of abused & neglected children; child development and individual and family functioning.
  • Ability to problem solve complex and possible crisis mental health client’s problems.

Crisis Therapist duties include but are not limited to:

  • Responds to pediatric behavioral health urgent care walk-in crisis situations and provides short term crisis stabilization.
  • Conducts in-person psychosocial assessments, risk assessments, AoD assessments, ACES assessments and safety/discharge planning with client and family.
  • Assesses level of care need and appropriate disposition planning.
  • Provides crisis counseling and de-escalation.
  • Provides support, education, counseling, de-escalation and service coordination to family members of client.
  • Links client and family to needed services, collaborates and acts as a liaison with local hospitals, providers and treatment team members.
  • Maintains accurate and up to date EHR documentation consistent with agency policies and procedures.
  • Works collaboratively with treatment team consisting of psychiatric provider, CPST, and Family Peer Support.
  • Receives ongoing supervision from Director of Family Stabilization.
  • Maintain licensure according to professional guidelines.
  • Attend agency and team meetings required. Participate in all team planning, case reviews, and program achievement.

  • Bachelor’s Degree, LSW/LPC.

Community Case Manager duties include but are not limited to:

  • Assess family and child needs and develop a treatment plan based on assessment and recording information to either treatment plan or BHSP.
  • Delivery of appropriate services, including; crisis mediation, case management, and coordination of community services.
  • Implement case management and supportive services per agency and program policy and procedures, including providing a written assessment.
  • Network with area services, understand eligibility requirements, make appropriate referrals, and link the family and child to community services to prevent ongoing case.
  • Determine need for additional services and make written request for an extension with justifications required.
  • Coordinates the treatment planning of all persons/agencies involved in an assigned case as indicated on the treatment plan or BHSP.

School/Community Case Manager Indirect Service:

  • Maintain licensure according to professional guidelines.
  • Attend agency and team meetings required. Participate in all team planning, case reviews, and program achievement.
  • Prepare and submit service reports according to requirements.
  • Maintain documentation of all case activity, contacts, goal plans, referrals, and other relevant family information per program and agency requirements.
  • Adheres to agency personnel policy and procedures, Ohio MHSA standards and fulfills documentation/reporting and productivity requirements.

Case Manager Minimum qualifications:

  • Possess a Bachelor’s/Master’s Degree in Human Services or related field.
  • Have prior experience working with children and families providing supportive services, crisis intervention, and family development for at-risk families.

  • Job duties will take place within school settings.

  • Counsels clients after selecting therapeutic approach to use with individuals, families and/or groups with clearly established measurable goals/objectives, frequency, intensity, location and duration of treatment.
  • Provide school consultation services.
  • If qualified, School Based Therapists may administer and interpret intelligence, achievement, interest and personality to diagnose disorders and formulate plans of treatment.
